PRIMARY REQUIREMENTS

Duly filled-up visa application form (computerized; font style and size must be ARIAL 12) (can be downloaded below) (Application form should be filled out all items correctly, If item is not applicable, please fill in [N/A]. The Embassy may not accept an application with blank items, no signature, or no date in the form; it may deny the application that is filled out incorrectly or contains incorrect information. Handwritten is not accepted.)

Photo - (TWO COPIES OF 4.5cm x 3.5cm or 2in x 1.4 in PHOTO With white background, taken within 6 months. Photo must be pasted on the application form. ) Must be in passport size no teeth shown,

Both ears must be visible, no accessories.

Original passport (at least 6 months valid beyond intended stay)

Copy of passport data page

Original and copy of valid visas and arrival stamps to OECD member countries for the past 5years

Original personal bank certificate Ideal amount for the current balance is P50,000-P100,000 (must include account type, current balance, account opening date, 6 months average daily balance)

Original bank statement or certified true copy of the passbook for the last 3 months

Copy of recent personal income tax return (must provide an affidavit if ITR cannot be produced)

ADD’L REQUIREMENTS IF EMPLOYED:

Original employment certificate (must include applicant’s position, date hired, compensation, office address and landline numbers, and e-mail address)

Copy of PRC/IBP IF (if professional)

Copy of company ID

Copy of business registration (DTI OR SEC)

Copy of business permit or mayor's permit
