may deny the application that is filled out incorrectly or wrong information.

TWO COPIES OF 4.5cm x 3.5cm or 2in x 1.4 in PHOTO

With white background, taken within 6 months Photo must be pasted on the application form.

BIRTH CERTIFICATE

MARRIAGE CERTIFICATE (if the applicant is married)

NOTE :

Must be issued within one year from PSA Main Office/Serbilis Outlet Center. *If the birth certificate from Philippine Statistics Office (PSA) is unreadable, or has incomplete

information, please submit the birth certificate issued by PSA together with a birth certificate

issued by the Local Civil Registrar. *If the birth certificate is “late registration”, please submit in addition, a baptismal certificate

and school record (Form 137) from high school or elementary and School Yearbook (if possible). *If there is no record in PSA, please submit a “Certificate of Non-Record” from PSA together

with a birth certificate from the Local Civil Registrar.

No ITR – kindly provide a notarized affidavit explaining why there’s no ITR

If ITR is not available and there is no ADB (Average Daily Balance) on Bank Certificate, submit a Bank

Statement to prove transaction for the last 3 months. (Ideal amount for the Average Daily Balance is

P50,000)

IMPORTANT REMINDERS:

Visa approval is at discretion of the Visa Officers. We cannot guarantee 100% Approval.

Visa submission is within 3 working days after the receipt of complete documents and if

there are no additional requirements needed. We strongly suggest applying at least 1

month prior to travel date because there are times where the visa officer is requesting for

additional documents.

All submitted documents are considered as FILED and no refunds shall be made in case

youmay have a change of mind on your application. For denied application, you can re-apply after 6 months. No Refunds shall be made

for the Visa Assistance fee regardless of the decision made on your application.
