PRIMARY VISA REQUIREMENTS

Copy of your Philippine Passport including the photo page of your passport and any pages

with amendments, endorsements, visas and entry / exit stamps.

One (1) passport-sized photo (head and shoulders only against a plain background and must

be taken within 6 months)

Birth certificate issued by the Philippine Statistics Authority (PSA)

Marriage Certificate (If applicable)

Bank certificate showing the average daily balance for the last three (3) months

Bank certificate showing the (6) months average daily balance.

ADD’L REQUIREMENT IF EMPLOYED:

Original employment certificate with leave of

absence (must include applicant’s position,

date hired, salary, date of return to work)

Copy of PRC/IBP ID (if professional)

Copy of company ID

3 months pay slip

ADD’L REQUIREMENT IF BUSINESS OWNER:

Copy of business registration (DTI or SEC)

Copy of business permit or mayor’s permit

Copy of business tax return for the last 2 years and deposit slip as evidence of tax payments.

Copy of financial statements.

Proof of enrollment

Copy of school ID

Leave of absence from school (if travelling while classes are ongoing)

For children travelling with one parent:

*Duly accomplished Form 1229 by non-travelling parent, and parent’s contact information and

valid government-issued ID with signature.

For children travelling with someone other than a parent:

*Duly accomplished Form 1229 by both parents, and both parents’ contact information and

valid government-issued ID with signature

For children travelling with someone other than a parent:

IMPORTANT REMINDERS:

Visa approval is at the discretion of the Visa Officers. We cannot guarantee 100% Approval.

Visa submission is within 3 working days after the receipt of complete documents and if there are no additional requirements needed.

We strongly suggest applying at least 1 month prior to travel date because there are times where the visa officer is request for additional documents.

All submitted documents are considered as FILED and no refunds shall be made in case you may have a change of mind on your application.

For denied application, you can re-apply after 6 months. No Refunds shall be made for the Visa Assistance fee regardless of the decision made on your application.
