Duly filled-up visa application form (signed)

Two passport sized colored picture with white back ground (must be taken within the last six

months; do not staple the photos in the form)

Original passport (At least 6 months valid beyond intended stay)

Original old passport (if with previous travel or visa)

Copy of passport data page

Copy of roundtrip airline reservation

Copy of hotel reservation

Original personal bank certificate

Original bank statement or certified thru copy of passbook for the last 6 months

Copy of recent personal income tax return

PSA Birth certificate and/or marriage certificate (if travelling with immediate family)

ADD’L REQUIREMENTS IF EMPLOYED:

Original employment certificate with leave of

absence (must include applicant’s position,

date hired, compensation, office address and

landline numbers and e-mail address)

Copy of company ID

Copy of PRC/IBP ID (if professional)

ADD’L REQUIREMENTS IF BUSINESS OWNER:

*Copy of business registration (DTI or SEC)

Copy of business permit or mayor’s permit

Copy of income tax return

Original school certification

Copy of School I.D.

Copy of parent’s document such as original bank

certificate, original COE or business documents

IMPORTANT REMINDERS:

Visa approval is at discretion of the Visa Officers. We cannot guarantee 100% Approval.

Visa submission is within 3 working days after the receipt of complete documents and if

there are no additional requirements needed. We strongly suggest applying at least 1

month prior to travel date because there are times where the visa officer is requesting for

additional documents.

All submitted documents are considered as FILED and no refunds shall be made in

case you may have a change of mind on your application.

For denied application, you can re-apply after 6 months. No Refunds shall be made

for the Visa Assistance fee regardless of the decision made on your application.
